  • State Board of Medical Examiners. Contact the State Board of Medical Examiners if you have a complaint about a physician in New Jersey. The Board licenses medical doctors in New Jersey and takes complaints very seriously. This site provides information about the complaint procedure and the Board’s privacy protections. It also contains a complaint form that may be completed online or mailed to the Board before a formal review of the complaint can begin. If you cannot complete the form online or print it for mailing, you may call the Board of Medical Examiners at 609-826-7100 and ask that the form be mailed to you.

  • The Division of Consumer Affairs in the New Jersey Department of Law and Public Safety provides an interactive list of health-professionals’ boards of examiners and advisory committees. Each link briefly describes the particular board’s responsibilities and provides contact information for consumers. Refer to this list if you need to file a complaint about a medical professional in New Jersey.
